SHN5123 - Understanding Community Care

Objectives:

Assessment tasks are designed to enable students to demonstrate the Learning and Employability outcomes for the relevant level of study. Level Learning Outcomes are embedded in the assessment task(s) at that level. This enables a more integrated view of overall student performance at each level.

Content:

In this module, you will explore the basis of community care, the ways in which community health services are organised and driven by local, national and global perspectives, the forms they take and could take, and tackle some of the challenges faced by these services. Particular attention will include but will not be limited to strengthening the challenges community health services face around issues such as homelessness, violence, addiction, mental health, disabilities, ageing, gender, racial and ethnic inequalities and prison health. Students will be encouraged to explore ways to engage with and strengthen community health services as well as opportunities to develop insight into the wide array of professions that an interest in community health services brings to health and social care practice. You will evaluate and develop skills relating to strengths-based approaches, motivational interviewing, health literacy, shared decision making, supported self-management, advocacy, restorative practice, behaviour change and cultural competence. You will critically review integrated care systems and interdisciplinary working to engage communities to improve their health and wellbeing.

Further Details Relating to Assessment
Post session activities submitted through Moodle will facilitate ongoing formative assessment opportunities via lecturer or peer feedback, these may include responses to case studies, quizzes, forum posts or blogs and self-assessment. Formative feedback will be provided through a series of tutorials and group tutorials.

Assessment 1: Group Presentation: You will work collaboratively in group to create a presentation (negotiated style) which demonstrates how strengths-based approaches can be used to support diverse communities to achieve positive health and wellbeing outcomes including asset based. The group will present to peers and defend it with Q & A.

Assessment 2: Literature Review (2500 words): you will critique the evidence base for the effectiveness and appropriateness of community interventions on individual's health and wellbeing including methods of assessing, planning and evaluating outcomes.
