Assessment tasks are designed to enable students to demonstrate the Learning and Employability outcomes for the relevant level of study. Level Learning Outcomes are embedded in the assessment task(s) at that level. This enables a more integrated view of overall student performance at each level.
The module will cover a range of long-term conditions (LTC), what they are and examples of key LTCs, the risk factors for the conditions, and the impact these conditions have on health and wellbeing. Interventions such as shared decision-making, supported self-management, living well with LTC, patient activation, social prescribing, improving health literacy, symptom management of conditions, the place of therapeutic relationships, telehealth, and a consideration of the place of psychosocial care will be covered. A strong focus will be placed on exploring how current public health strategies and campaigns aim to reduce health inequalities in vulnerable groups living with LTCs. You will also be encouraged to actively consider how health and care professionals can work to design plans, strategies, and information resources that support how to communicate person-centred care strategies effectively to service users who need to manage the impact that long-term conditions have on health and well-being.

Post session activities submitted through Moodle will facilitate ongoing formative assessment opportunities via lecturer or peer feedback, these may include responses to case studies, quizzes, forum posts or blogs and self-assessment. The opportunity for formative feedback will be provided by the module leader.
The information leaflet will be based on one long term health condition, covering the management and treatment strategies related to supporting persons with the named LTC. The information leaflet may include statistics, images, and text as deemed relevant to the condition in focus. The information leaflet will need to be fully referenced.
The assessment and care support plan will involve students choosing an individual with a named LTC from a range of case studies. Students will be required to discuss how the individual’s need for care and support has been assessed and outline a care plan that includes what the care and support for this individual should look like.
The assessments will allow you to demonstrate knowledge and understanding of concepts and theories relevant to public health approaches and interventions for supporting people with long-term conditions.
