Assessment tasks are designed to enable students to demonstrate the Learning and Employability outcomes for the relevant level of study. Level Learning Outcomes are embedded in the assessment task(s) at that level. This enables a more integrated view of overall student performance at each level.
The module will cover principles and practice of personalised care, including the holistic provision of care, assessment and support planning for individuals and communities. The role of social prescribing as a fundamental intervention for the universal model of personalised care will be central to your study. You will evaluate and develop skills relating to strengths-based approaches, motivational interviewing, health literacy, shared decision making, supported self-management, advocacy, restorative practice, behaviour change and cultural competence. You will critically review integrated care systems and interdisciplinary working to engage communities to improve their health and wellbeing, taking account of the role of a social prescriber.

Post session activities submitted through Moodle will facilitate ongoing formative assessment opportunities via lecturer or peer feedback, these may include responses to case studies, quizzes, forum posts or blogs and self-assessment. Formative feedback will be provided through a series of tutorials and group tutorials.
Assessment 1: Group Presentation: You will work collaboratively in group to create a presentation (negotiated style) which demonstrates how strengths-based approaches can be used to support diverse communities to achieve positive health and wellbeing outcomes including asset based and social prescribing methods. The group will present to peers and defend it with Q & A.
Assessment 2: Literature Review: you will critique the evidence base for the effectiveness and appropriateness of social prescribing interventions on individual's health and wellbeing including methods of assessing, planning and evaluating outcomes. 2500 words. You will ensure you use a range of research methodologies.
The assessment tasks will allow you to demonstrate knowledge, understanding and application of the principles of personalised care and social prescribing.
