Assessment tasks are designed to enable students to demonstrate the Learning and Employability outcomes for the relevant level of study. Level Learning Outcomes are embedded in the assessment task(s) at that level. This enables a more integrated view of overall student performance at each level.
The module will cover principles and practice of personalised care, including the holistic provision of care, assessment and support planning for individuals and communities. The role of social prescribing as a fundamental intervention for the universal model of personalised care will be central to your study. You will evaluate and develop skills relating to strengths-based approaches, motivational interviewing, health literacy, shared decision making, supported self-management, advocacy, restorative practice, behaviour change and cultural competence. You will critically review integrated care systems and interdisciplinary working to engage communities to improve their health and wellbeing, taking account of the role of a social prescriber.

Post session activities submitted through Moodle will facilitate ongoing formative assessment opportunities via lecturer or peer feedback, these may include responses to case studies, quizzes, forum posts or blogs and self-assessment. One draft opportunity will be provided for each summative assessment with feedback provided at least one week prior to submission.
Group Presentation: You will work collaboratively in group to create a presentation (negotiated style) which demonstrates how strengths-based approaches can be used to support diverse communities to achieve positive health and wellbeing outcomes including asset based and social prescribing methods. The group will present to peers and defend it with Q & A.
Literature Review: you will critique the evidence base for the effectiveness of social prescribing interventions on individual's health and wellbeing including methods of assessing, planning and evaluating outcomes. 2000 words.
Oral Assessment: you will conduct a mock personalised care session for 15 min demonstrating motivational interviewing skills to support a client to develop a therapeutic plan. You will provide supporting notes for your preparation of the session and a post session reflection evaluating your performance. You will work with a fellow Level 5 student who will role play the part of your service users, you will have the opportunity to brief your partner during a pre planning session to outline the focus of the session and your expectations of their input during the session.
The assessment tasks will allow you to demonstrate knowledge, understanding and application of the principles of personalised care and social prescribing.
