Assessment tasks are designed to enable students to demonstrate the Learning and Employability outcomes for the relevant level of study. Level Learning Outcomes are embedded in the assessment task(s) at that level. This enables a more integrated view of overall student performance at each level.
The module will cover a range of long-term health conditions that may include and is not limited to the following long-term conditions (LTC); stroke, heart disease, kidney disease, type 2 diabetes or dementia. A basic overview of the body systems and links to the LTCs will be taught integrally to the conditions most relevant to the current population of the UK which will include; risk factors/lifestyle, treatment and self-management strategies. A strong focus will be placed on public health prevention; these methods will be continued into modules at Levels 5 and 6. You will be introduced to using health intelligence data to understand incidence and prevalence of LTCs and how these can be managed.

Post session activities submitted through Moodle will facilitate ongoing formative assessment opportunities via lecturer or peer feedback, these may include responses to case studies, quizzes, forum posts or blogs and self-assessment. One draft opportunity will be provided for each assessment with feedback provided at least one week prior to summative submission.
Exam: Short answer questions that requires you to demonstrate your knowledge and understanding of LTCs and the management and treatments of these conditions.
Portfolio: The portfolio will be based upon a case study. You will complete a task which is to choose a long-term condition covered in the module and produce an infographic using health data that can be displayed in a care setting; this will cover the management and treatment of the LTC.
The assessments will allow you to demonstrate knowledge and understanding of concepts and theories relevant to public health approaches and interventions for supporting people with long-term conditions.
