SHN4033 - Supporting People with Long-term Conditions

Objectives:

Assessment tasks are designed to enable students to demonstrate the Learning and Employability outcomes for the relevant level of study. Level Learning Outcomes are embedded in the assessment task(s) at that level. This enables a more integrated view of overall student performance at each level.

Content:

The module will cover the following long-term conditions (LTC); stroke, heart disease, kidney disease, type 2 diabetes, dementia and respiratory disease. An applied physiology approach will be taken covering the body systems of the main conditions, aetiology, pathology, risk factors/lifestyle, treatment, and self-management strategies. A strong focus will be placed on public health prevention i.e., screening through the NHS Health Check and policy and strategy to reduce health inequalities in vulnerable groups. Interventions such as shared decision making, supported self-management, living well with LTC, patient activation, social prescribing and improving health literacy will be covered. You will also be introduced to using health intelligence and data to understand incidence and prevalence of LTC i.e., morbidity and mortality rates.

Post session activities submitted through Moodle will facilitate ongoing formative assessment opportunities via lecturer or peer feedback, these may include responses to case studies, quizzes, forum posts or blogs and self-assessment. One draft opportunity will be provided for each assessment with feedback provided at least one week prior to summative submission.

Exam: Short answer questions that requires you to demonstrate your knowledge and understanding of applied physiology across a range of body systems and conditions.

Portfolio: The portfolio will be based upon an extended case study. You will complete 2 tasks including (1) chose a long-term condition covered in the module and produce an infographic using health data that can be displayed in a GP waiting room (2) report – discuss how the NHS Health Check and personalised care can support the prevention and self-management of long-term health conditions.

The assessments will allow you to demonstrate knowledge and understanding of concepts and theories relevant to public health approaches and interventions for supporting people with long-term conditions.
